Listed are the Appropriations Forms that must be filled out and submitted to our office. The first form is for all projects except Defense. The second form is for Defense related requests only.
Please fill out each form completely. If you have a question about any section or request, please contact us and we will help you figure out the best way to answer.
DO NOT LEAVE ANY BLANK SECTIONS. ANY INCOMPLETE FORMS COULD AFFECT THE APPROVAL OF THE PROJECT.
Include any relevant information that might help the committee when deciding which projects to include:
- Any non-governmental funding
- Any non-federal governmental funding
- Any prior year federal funding
- This funding can include in-kind funding that would be part of the budget; i.e. rent on rooms to be used, or utility costs that are paid by an outside group
You must also provide a letter on your stationary that contains your projects in priority order. Each project must include a 1 paragraph description. In keeping with the spirit of the Appropriations Committee commitment to provide unprecedented transparency to the appropriations process please include information explaining the purpose of the earmark and why it is a valuable use of taxpayer funds. Please include this information in a separate un-scanned Word document as well.
Appropriation requests must be submitted to our office no later than 5:00 p.m. February 27, 2009.
