Circle of Life Children - Expansion of Pediatric Palliative Care Team
 
Amount Requested: $75,000
Circle of Life Children’s Center provides palliative and end of life care for children pre-birth to age 21 who are diagnosed with a chronic, progressive, life-limiting illness or medical condition.  Families are provided with education regarding illness, assistance in effectively communicating with healthcare professionals, palliative care options, setting goals of care, guidance for difficult decisions, referrals to community resources, home visits, counseling and bereavement support. This project will underwrite the cost to provide families with information and counseling in their own language and urgent aid through community resources as needed.
