Jewish Family Service of MetroWest - Health Intervention and Homelessness Prevention Program
 
Amount Requested: $300,000
This project will provide support to adults who live with chronic mental illness, reducing their reliance on taxpayer funded residential resources.  The goal is to assist clients with maintaining the highest level of independence, minimizing hospitalizations and maintaining stability in their everyday living.
